The Cardiac Wellness Center
The Cardiac Wellness Center is located in our Corporate Woods office and offers physician-directed exercise, nutrition and cardiac disease risk-management programs and educational seminars for all ages. Programs are adapted from cardiac rehabilitation programs developed to help patients with coronary artery disease resume a normal lifestyle and reduce risk of disease progression following a heart attack, angina or heart surgery.
A physician referral is required for all programs. A stress test may be required before
beginning Phase II exercise programs.

How To Request Lab Results, Test Results and Prescription Renewals
To better serve our patients, we have created a unique voice mailbox system that allows patients to obtain lab results, diagnostic test results, and even prescription renewals over the telephone. The system is easy to use and saves you time and the need to make multiple phone calls.

Medical Insurance
Capital Cardiology Associates, PC participates in most medical insurance plans. When making your first visit to our office, please be sure to bring your insurance ID card, a referral from your primary care physician (if required), and any prescription medications you may be taking.
Your co-pay and any balance not covered by your insurance is due at the time of your office visit. There is an $8.00 service fee if the co-pay or uninsured balance is billed to your home.
Patient Records
In order to obtain a copy of your patient records, you will need to sign a release form, which is available at each office. Please allow five business days to process your request.
