• For inpatients in any hospital who have to be transferred to either Albany Medical Center or St. Peter's Hospital to undergo an invasive cardiac procedure, such as: angioplasty, artherectomy, coronary artery stenting, cardiac catheterization, electrophysiology testing, catheter ablation, and defibrillator or pacemaker implantation.
  
  
• For outpatients who need to be scheduled for an invasive cardiac procedure.
  
  
• For inpatients who have had an invasive cardiac procedure completed and need to be transferred back to their community hospital and physician.

Transfer Center Coverage

Transfers may be initiated 24 hours a day by calling the Transfer Center at 518-292-6040. The Transfer Center Team is available weekdays between the hours of 8:30 AM and 5:00 PM to make all necessary arrangements and appointments. After hours and during weekends, transfers are handled by the interventional cardiologist on call for Capital Cardiology Associates, PC.
